Jon Hilton, Precedence Health Care’s recent Director of Programs, passed away in July 2013. Jon was an exceptional person who had an extraordinary vision for a health care system enabled by internet and mobile technologies. He was a major contributor to the design and implementation of the cdmNet chronic disease management service and championed its adoption across Australia. To that end, Jon benefited from a cdmNet care plan himself. His journey is documented in a video produced by the Health Informatics Society of Victoria.
