Health Care Homes represent the largest digital health program initiated by Australia’s Department of Health to tackle the growing epidemic of chronic illness. Close to 200 Health Care Homes around Australia, including Aboriginal Community Controlled Health Services, will provide better coordinated care for up to 65,000 Australians who are living with chronic and complex conditions.
The program relies on PracticeProfiler to identify patients most in need of care and to assess their care needs. Configured for Health Care Homes, PracticeProfiler’s smart predictive technology—validated by CSIRO— determines which patients in a doctor’s practice are most likely to have a potentially preventable hospitalisation in the coming year. PracticeProfiler then determines what level of preventative care these patients need, based on a smart digital assessment completed by the patient’s GP in consultation with the patient.
NPS MedicineInsight uses PracticeProfiler for extracting de-identified patient data from GP clinical systems.
The MedicineInsight program provides guidance and direction on the safe and wise use of medicines and health technologies so that people stay healthier and the cost of care remains affordable.
To achieve this objective, MedicineInsight works with Precedence to collect and analyse clinical and other information obtained from primary care practices across Australia. Through its integration with GP clinical systems, PracticeProfiler securely provides the comprehensive clinical data MedicineInsight needs to deliver meaningful information for health consumers, health professionals, government, research and other businesses.
