Integration

The Inca Integrated Care Platform syncs with all major practice software to automatically populate key demographics and clinical information.

Care Plans

Inca creates Medicare-compliant GPMPs and TCAs. Our guided templates have been developed in consultation the RACGP and include input from key advisory agencies such as Diabetes Australia.

Shared Health Record

Real-time access allows care team members to communicate easily, and share reports, case notes and other patient details. This visibility allows for better-informed decision making which can lead to improved patient outcomes.

Patient Engagement

Patient engagement is a key contributor to the success of care plans. Inca provides patients with access to their personalised care plans and allows them to communicate with their care team via the Inca web portal. Our Meditracker App also allows them to access their health record at any time.

Who uses Inca?

GPs

Inca produces personalised digital care plans and automatically generates and distributes key documents such as allied health referral forms. This means that providers can spend more time with their patients and less on paperwork.

Allied Health and Specialists

Inca automatically shares the patient’s GP summary medical record, care plan, and My Health Record with allied health and specialists in the patient’s care team. This improves team collaboration and makes sure no one falls between the cracks.

Corporates

Inca can be customised to provide advanced features for organisational level care coordination, budgeting, reporting and task management.
