MediTracker

The MediTracker app displays the patient’s GP medical summary, retrieved from the practice’s clinical software, including past and current condition history, medications, allergies, immunisations, measurements and pathology results.  

Patients always have their medical record on hand, in emergencies, on travel, or simply out of GP hours. MediTracker also links with Apple Health, Google Fit, wearable and other wireless devices, such as blood pressure and blood glucose monitors.

‍MediTracker lets patients choose which healthcare providers to include on their care team and share their summary medical record. This ensures care team members have the necessary information for critical decisions, fosters collaboration, and empowers patients to take control of their health.
