Keep your high-risk patients out of hospital during COVID-19
Precedence Health Care (Precedence), supplier of the Inca Chronic Disease Management Platform, recently announced the Risk Stratification Tool, a new feature which will help general practitioners keep their high-risk patients out of hospital during COVID-19.
The new digital health feature comes at an ideal time as primary healthcare moves to virtual clinics, and telehealth services become the new norm for chronic disease management.
The Risk Stratification Tool (RST), based on the CSIRO developed algorithm, identifies patients and produces a report on those that are at highest risk of hospitalisation in the next 12 months. The RST is currently being used successfully in the Health Care Homes trials around the country.
Precedence General Manager Grant Williamson says that the launch of this new feature is of enormous benefit to the patient, medical practices and the health system during the current pandemic.
“It will assist busy general practitioners and their teams to identify high-risk patients, better manage their care and share the plan with the allied health team without printing, faxing, mailing or using secure messaging.
“Inca allows all carers of the patient to collaborate on our platform like an online multi-disciplinary clinic. The end result is to keep high-risk patients out of hospital and enable them to be managed within the safety of their own home.
The Inca platform supports the development of compliant and evidence-based care plans, allowing the GP to share the plan, along with an up-to-date and relevant health summary, current medications and results with the care team. All members of the team can then upload documents, reports, progress notes and share expertise where applicable.
Associate Professor Malcolm Clark, medical director of IPN Victoria, uses Inca extensively at his general practice in Camberwell and says that it is invaluable within the complex COVID-19 environment.
“Using the RST, you can immediately identify those patients who are at the highest health risk. After initially managing them via a telehealth consultation, we then use the Inca platform to share the care with the allied health team.”
