Access to cdmNet for GPs participating in the CKD Detection and Management in General Practice Project
Welcome to the Chronic Disease Management Service (cdmNet). cdmNet is an online service (“cdmNet Service”) for supporting general practices, other healthcare providers, and patients to better manage chronic disease.
A general practitioner (GP) may become a registered member (“Member”) of cdmNet at no cost. A Member has the ability to participate in some, but not all, of the features and services available within the cdmNet Service.
To access additional features and services, including the ability to prepare and complete certain MBS Chronic Disease Management (CDM) Items, the GP or a practice for which the GP provides chronic disease management services must agree to payment of the charges applicable to these additional services. This requires a GP Entity (an individual, practice, or other corporate entity) to agree to the cdmNet Service Agreement.
If you are participating in the Chronic Kidney Disease Detection and Management in General Practice Project being run by Macedon Ranges and North West Melbourne Medicare Local, up to 30 June 2014 you will not be charged any cdmNet service fees for patients identified with chronic kidney disease.
You (in your own right or having the authority of the GP Entity) can complete the cdmNet Service Agreement electronically by entering the information requested below and agreeing to the Terms and Conditions of the Service Agreement.
