cdmNet is an online service (“cdmNet Service”) for supporting general practices, other healthcare providers and patients to manage chronic conditions and preventive care. A general practitioner (GP) may become a registered member (“Member”) of cdmNet at no cost. A Member has the ability to participate in many, but not all, of the features and services available within cdmNet.

As part of CarePoint, participating GPs in your practice are entitled to receive, free of charge, access to the entire cdmNet service for all patients enrolled in the program.

Your practice may choose to use cdmNet for other patients not enrolled in CarePoint, however there may be fees for access to some of the services.

If your practice enrols at least 50 patients prior to the end of the CarePoint enrolment period (30 September 2015), your practice will be able to use cdmNet for all of your patients for free until 30 September 2016. For the avoidance of doubt, an enrolled patient means a patient who has an approved care plan in cdmNet.

To access cdmNet, your practice (GP Entity) must agree to the cdmNet Service Agreement.

To access additional features and services the GP or a practice must complete the service agreement below. This requires a GP Entity (an individual, practice, or other corporate entity) to agree to the cdmNet Service Agreement.

On completing this agreement a Member will have access to:

  • Assistance with registration, installation and set up
  • Personalised training
  • Access to the cdmNet Support Desk (telephone and email)
  • The ability to prepare and complete high-value MBS chronic disease management items, including GP Management Plans, Team Care Arrangements, Home Medicines Reviews, Health Assessments and Diabetes Annual Cycles of Care.

You (having the authority of the GP Entity) can complete the cdmNet Service Agreement electronically by entering the information requested below and agreeing to the Terms and Conditions of the Service Agreement.
