An easier way to edit the patient’s care team

New functionality in cdmNet gives you the option to directly add, remove or change care team members from the Care Team tab. This makes managing the care team so much easier!

To edit providers in the care plan please follow the steps below.

To Change a Provider

  • In the Care Team tab, navigate to the provider you would like to change and click on Change Provider
  • This will give you two options
    • Changing to a preferred provider – you can click on the name of a preferred provider from the list and your provider will be changed
    • Changing to another provider– click on Other Provider and search for the provider from cdmNet’s national directory of providers. Once you find the provider that you would like to change to, select their name and click on Add Selected Provider.

To Add a New Provider

  • In the Care Team tab, click on Add Provider on the right side in the Care Team tab.
  • This will give you the option to search from cdmNet’s national directory of providers. Once you find the provider you would like to add, select their name and click on Add Selected Provider.

Removing a Provider

  • In the Care Team tab, navigate to the provider you would like to remove and click on Remove from Care Team
  • This will remove the selected provider from the care team
